Job summary

Superior Hospice is hiring a Hospice RN Case Manager to provide one-on-one patient care, build strong relationships with families, manage your own caseload, and exercise clinical autonomy to deliver compassionate, respectful end-of-life care.

You will work within a 21-county Greater Twin Cities area service territory, with a full-time schedule including weekend and on-call rotations, and a strong emphasis on relationship-centered care and clinical excellence.

Qualifications

  • Active Minnesota RN license required.
  • Experience in hospice or related nursing preferred (oncology, ICU, palliative care, home health or similar).
  • Strong clinical judgment and the ability to work independently.
  • Excellent communication, organization and critical thinking.
  • Passion for relationship-centered, patient-focused care.
  • Reliable transportation and willingness to travel across the assigned territory.

Responsibilities

  • Manage your own hospice caseload as the primary RN for patients and families.
  • Complete comprehensive nursing assessments and develop individualized plans of care.
  • Provide expert pain and symptom management focused on comfort, dignity and quality of life.
  • Guide and educate patients, caregivers and families throughout the hospice journey.
  • Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists and community partners.
  • Provide care in patients' homes, assisted living communities and skilled nursing facilities.
  • Maintain accurate EMR documentation and ensure Medicare hospice compliance.
  • Participate in a weekend and on-call rotation for continuity of care.

What You'll Do
