Job summary

Superior Hospice is seeking a Hospice RN Case Manager to provide compassionate, one-on-one home-based care across the Greater Twin Cities metro area. You will manage an assigned caseload, assess needs, and coordinate with an interdisciplinary team.

This role offers autonomy, strong support from leaders, and the chance to guide patients and families through the hospice journey with dignity and comfort.

Qualifications

• Active Minnesota Registered Nurse (RN) license.
• Experience in hospice, oncology, ICU, palliative care, home health, or another high-acuity clinical setting.
• Strong assessment skills, sound clinical judgment, and the ability to work independent.
• Passion for relationship-centered, patient-focused care.
• Excellent communication, organization, and critical thinking skills.
• Reliable transportation and willingness to travel throughout the service area.

Responsibilities

• Manage an assigned hospice caseload while serving as the primary nurse for patients and families.
• Complete comprehensive nursing assessments and develop individualized plans of care.
• Deliver expert pain and symptom management focused on comfort, dignity, and quality of life.
• Educate patients, caregivers, and families throughout every stage of the hospice journey.
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and other interdisciplinary team members.
• Provide care in private homes, assisted living communities, and skilled nursing facilities.
• Maintain accurate, timely EMR documentation while ensuring compliance with Medicare hospice regulations.
