Job summary

Hospice of South Texas seeks an RN Case Manager to deliver comprehensive nursing care in the Victoria/Port Lavaca area. The role includes quick assessment, care planning, and patient-family education on disease process, nutrition, and end-of-life needs.

The position requires two years of RN experience (hospice or home health preferred) and a valid RN license with a driver’s license. On-call and weekend duties are part of the schedule.

Qualifications

• Graduate from an accredited Diploma, Associate or Baccalaureate Nursing program.
• Current State license as a Registered Nurse.
• Valid State Driver's License.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients.
• Implement the individualized Plan of Care and revise as needed.
• Educate patients and families about disease process and end-of-life care.
