The Care Team Hospice is seeking a compassionate RN Case Manager to coordinate patient care, manage plans of care, and collaborate with physicians and the interdisciplinary team to ensure comfort and quality of life.

This on-site role supports a Monday–Friday schedule with on-call opportunities and a collaborative, growth-focused environment that values clinical excellence and compassionate end-of-life care. Join us in delivering exceptional care for patients wherever they call home.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support positive outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care.
