Qualifications

• Missouri or Compact RN License.
• Valid driver's license and auto liability insurance.

Responsibilities

• Educate patients and family about the plan of care and guide healthcare decisions.
• Consult with the attending physician and Hospice Medical Director regarding changes to patient condition or plan of care.
• Coordinate hospice care with the team and family to meet needs.
• Perform initial and ongoing comprehensive assessments.
• Conduct hospice care evaluations and informational visits.
