Job summary

Transitions Hospice is seeking an RN Case Manager to serve patients in Oak Park, Cicero, and surrounding areas. You will coordinate care, guide the interdisciplinary team, and ensure timely documentation while delivering compassionate, patient-centered services.

The role requires an active RN license in Illinois, graduate nursing degree, and ideally 1+ year hospice/home health experience. You will work 8 AM–8 PM with an alternating 4/3 schedule including weekends, and participate in the on-call

Qualifications

Active RN license in the state of employment.
Graduate of an accredited school of Nursing.
1+ year Hospice or home health experience preferred.
Strong clinical judgment and the ability to work independently in the field.
Excellent communication skills across care teams and families.

Responsibilities

Conduct comprehensive ongoing assessments to understand patients' evolving needs.
Build and adjust personalized care plans with physicians and care teams.
Administer medications and treatments per physician orders.
Keep clinical documentation accurate, timely, and meaningful.
Serve as the communication bridge between families, physicians, and community health partners.
Educate patients and caregivers so they feel confident and supported.
Participate in on-call rotation per team policy.
