Job summary

Companion Hospice in Guthrie, OK is seeking a dedicated RN Case Manager to lead patient-centered end-of-life care. You will develop care plans, coordinate services across settings, and supervise aides, ensuring compassionate, high-quality support to patients and families.

You will educate families on the Medicare Hospice Benefit, manage documentation, and collaborate with the interdisciplinary team to deliver dignified care with autonomy and leadership support.

Qualifications

• Current Oklahoma Registered Nurse license.
• At least 1 year of medical/surgical, oncology, home health, or hospice experience.
• Strong knowledge of pain and symptom management in terminally ill patients.

Responsibilities

• Develop and manage individualized Plans of Care with patients, families, physicians, and the IDG.
• Coordinate hospice services and ensure seamless care across settings.
• Provide expert pain and symptom management.
• Educate patients and families on the Medicare Hospice Benefit and hands-on care.
• Supervise Home Health Aides and deliver high-quality care.
• Complete timely documentation and participate in admissions.
