Job summary

Tranquility Hospice is seeking an experienced RN Case Manager to coordinate comprehensive hospice care in Marion, Delaware OH, and surrounding counties. You will assess patient and family needs and implement the interdisciplinary plan of care.

Expect leadership responsibilities, on-call rotation, and collaboration with the IDG. Travel to service counties and supervision of STNA/LPN staff are part of the role. A valid Ohio RN license is required.

Qualifications

• Minimum of one year RN experience.
• Hospice, Home Health, or Long-Term Care experience preferred but not required.
• Strong leadership and organizational skills.
• Effective assessment and critical thinking skills.
• Knowledge of the case management processes.
• Valid Driver’s License with reliable, insured automobile.
• Must be able to communicate well, both verbally and in writing.

Responsibilities

• Plan, coordinate and provide hospice care in accordance with the patient's plan of care; coordinate with team members and family to meet needs.
• Submit complete, accurate and relevant clinical notes in a timely manner; participate in IDG and case conferences.
• Provide education to patients and families regarding the dying process and care plans.
• The RN case manager plans and delivers care using the nursing process (planning, interventions, implementation, evaluation).
• Provide supervision and education to STNA’s and LPN’s.
• Participate in bi-weekly interdisciplinary team meetings.
• Develop and update the plan of care as patient needs change.
• Provide education to patients, families, and referral sources.
• Identify resources and referrals (OT, PT, Pharmacy, DME, etc.).
• Communicate shift reports effectively in writing and verbally.
• Utilize Electronic Health Record, Email, computers, and cell phones.
