Job summary

Absolute Hospice is seeking an experienced, compassionate RN to join our hospice team in Massillon, OH. The RN Case Manager will provide direct patient care and support to families, coordinating care with an interdisciplinary group and managing patient records.

Responsibilities include assessment, development and implementation of the plan of care, education of patients and families on medications, dressing changes, oxygen and hospice philosophy, and ensuring high-quality care in line with the

Qualifications

• Licensed RN with current state license.
• Experience in hospice or end-of-life care preferred.
• Ability to educate patients and families and coordinate care.

Responsibilities

• Provide direct patient care to hospice patients and support to families.
• Manage an interdisciplinary team and care staff to enhance quality of hospice care.
• Educate patient, family or caregivers including medications, dressing changes, oxygen, and hospice philosophy; document care in Plan of Care.
