Job summary

Tranquility Hospice is seeking a compassionate Registered Nurse (RN) to provide patient-centered hospice care across counties in Ohio. You will coordinate the interdisciplinary team and support patients and families through the dying process.

This full-time role requires strong leadership, communication, and case-management skills, with on-call rotation and travel as needed. Join a supportive team dedicated to comfort, dignity, and high-quality end-of-life care.

Qualifications

• Must have an active Ohio RN license.
• Experience in hospice, home health, or long-term care preferred.
• Strong leadership and organizational skills.

Responsibilities

• Plan, coordinate, and provide hospice care per patient care plans.
• Collaborate with patients, families, and team members to meet needs.
• Consult with attending physician and Hospice Medical Director on changes.
• Submit complete, accurate clinical notes and participate in IDG/case conferences.
• Educate patients and families about the dying process and care plans.
• Travel to service counties as required.
• Supervise and educate STNA’s and LPN’s.
• Participate in bi-weekly interdisciplinary team meetings.
