Gentiva Hospice is seeking a Hospice Registered Nurse to lead patient care in home settings across Lenexa, KS. The RN Case Manager will assess, plan, educate, and coordinate with an interdisciplinary team to deliver compassionate end-of-life care.

You will educate families, document care, and ensure regulatory compliance while supporting patients and loved ones with dignity, comfort, and emotional support.

Qualifications

• 1+ year RN experience in hospice, home health or related settings.
• Hospice or palliative care experience preferred.
• Current state RN license.
• CPR certification.
• Valid driver’s license and auto insurance.

Responsabilités

• Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
• Deliver and document skilled, hands-on nursing care based on each patient’s Plan of Care in home or equivalent settings.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with an interdisciplinary team to build and refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones; educate caregivers.
