This full-time role involves building care plans, managing clinical direction for interdisciplinary teams, and ensuring quality patient care. Candidates must have a nursing degree, current RN license, and two years of relevant experience. Qualifications

• Current State license as a Registered Nurse.
• Current CPR certification.
• Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred.
• Must be a licensed driver with an insured automobile.
• Able to lift, position and/or transfer patients and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team.
• Implement, develop and document the patient plan of care.
• Provide care utilizing infection control measures.
• Monitor assigned cases for compliance with third-party payer requirements.
• Responsible for supervising hospice aides and licensed practical nurses.
