Qualifications
Current State license as a Registered Nurse.
Current CPR certification.
Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred.
Must be a licensed driver with an insured automobile.
Able to lift, position and/or transfer patients and equipment.
Responsibilities
Case manages and provides clinical direction to the interdisciplinary team.
Implement, develop and document the patient plan of care.
Provide care utilizing infection control measures.
Monitor assigned cases for compliance with third-party payer requirements.
Responsible for supervising hospice aides and licensed practical nurses.
