Job summary

Amedisys is seeking a Hospice RN Case Manager in Williamsburg, VA to assess patients, collaborate with the care team, and implement individualized care plans.

Qualifications

• One year of nursing experience.
• RN license valid for the state(s) assigned.
• Current CPR certification and valid driver’s license.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice disciplines as needed or document rationale.
• Maintain individualized patient care progress records and document pain/symptom changes.
• Educate patients, families, and caregivers on care techniques and plans.
• Communicate regularly with the clinical manager and care team; participate in on-call coverage.
