Zusammenfassung

Amedisys Home Health is seeking an RN Hospice Case Manager to make a difference every day in the home. You will assess patients, coordinate hospice care, and collaborate with the care team to implement a plan of care.

We offer attractive pay ($84,000–$87,000) plus mileage, a $5,000 sign-on bonus, and benefits starting the first of the month after hire. Choose PPO or HSA medical plans with telehealth and PTO.

Qualifikationen

• At least one year of nursing experience.
• Current RN license valid for assigned state.
• Current CPR certification.
• Valid driver’s license with reliable transportation and liability insurance.

Aufgaben

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Kenntnisse

One year of nursing experience

Current RN license
