Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our team delivering compassionate hospice and home health care. You will assess patients and families, monitor conditions, and coordinate with physicians and care teams to implement the plan of care.

Requirements include a current RN license, graduation from an approved nursing program, at least 1 year of clinical experience, and the ability to lift up to 50 pounds.

Qualifications

• Current State license as a Registered Nurse is required.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.
• Experience in hospice or home health is preferred.

Responsibilities

• Assess hospice patients and families to identify needs (physical, psychological, social, spiritual, environmental).
• Monitor patient condition and communicate changes to physician and care team.
• Educate and support patients, families, and caregivers in following the Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments.
