Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our elite team delivering right care at the right time in patients' homes. The role involves assessing needs, coordinating with physicians and care teams, and educating patients, families, and facility staff.

Responsibilities include monitoring patient conditions, communicating changes, and providing bereavement support as part of a compassionate, interdisciplinary Plan of Care.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical setting.
• Ability to lift up to 50 pounds and move equipment.
• Valid driver’s license or reliable transportation to travel to patient homes.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families for physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and report changes to physician and care team.
• Educate and support patients, families, caregivers, and staff in the Plan of Care.
• Provide education on hospice philosophy, pain and symptom management to staff in care settings.
• Provide compassionate support at the time of death and complete bereavement assessments.

Education

Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
