Qualifications: valid RN license for the state, CPR certification, 1+ year of nursing experience, and a valid driver’s license with reliable transportation. Salary ranges from $75,620 to $113,430 annually, with comprehensive benefits.

• Current CPR certification.
• At least 1 year of nursing experience.
• Valid driver’s license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
