Job summary

Amedisys seeks a Hospice Registered Nurse Case Manager for Beacon Hospice. You will assess patients, collaborate with the care team, and implement care plans in a home-based hospice setting.

Qualifications

• One year of nursing experience.
• Current RN license, state of assignment.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain and symptom management changes/outcomes.
• Educates patients, families, and caregivers on care techniques.
• Communicates with clinical manager and care team.
• Performs on-call responsibilities as assigned.
• Other duties as assigned.
