Job summary

Amedisys, part of the Optum family, is hiring an RN Case Manager to coordinate compassionate hospice care. You will assess patients, partner with the interdisciplinary team, and develop care plans tailored to individual needs.

Qualifications

• 1+ years of nursing experience.
• Current RN license valid for the assigned state.
• Current CPR certification and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice disciplines as needed or documents rationale for not doing so.
• Maintains patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes and outcomes.
• Educates patients, families, and caregivers on care techniques.
• Communicates with clinical manager and care team regularly.
• Performs on-call responsibilities for patients/families as assigned.
• Other duties as assigned.
