Qualifikationen

• RN license in state required.
• Bachelor's degree in Nursing preferred.
• Minimum 1 year of acute care experience in an institutional setting.

Aufgaben

• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan that establishes goals, based on nursing diagnosis and becomes part of palliative actions.
• Provides health care instructions to the patient per assessment and plan.
• Assists the patient with activities of daily living and supports self-sufficiency and comfort care.
• Acts as Case Manager when assigned by Clinical Manager and coordinates patient care for caseload.
• Communicates with the physician regarding patient needs and reports changes in condition; obtains orders as required.
• Teaches patient and family self-care techniques and provides medication/diet instructions as ordered by physician.
