Job summary

As part of a collaborative care team, you will relieve suffering, provide education to families, and document care plans with a patient-first approach. A valid RN license and willingness to travel are essential for success.

Qualifications

• Current, unrestricted Registered Nurse (RN) license.
• Minimum of one year of nursing experience.
• Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area.

Responsibilities

• Provide high-quality bedside care to patients and families in home settings, assisted living, and SNFs.
• Deliver compassionate care using hospice philosophy and clinical expertise.
• Manage patient symptoms with clinical judgement and empathy.
• Coordinate with patients and families to support the hospice journey and ensure comfort.
