Qualifications

Responsibilities

• Make the initial nursing evaluation for hospice services.
• Identify patient/family needs and reassess regularly.
• Initiate and coordinate the plan of care.
• Document patient problems and goals.
• Collaborate with team members and provide daily care.
• Instruct and supervise self-care techniques.
• Maintain clinical notes on the patient's condition.
