Qualifications

• Current Indiana RN license.
• Michigan RN license is a plus.
• Strong clinical assessment, communication and critical‑thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Knowledge of hospice philosophy, applicable coverage requirements, and state and federal regulations is preferred.

Responsibilities

• Assess patient’s physical, psychosocial, environmental, safety and spiritual needs.
• Develop and manage individualized plans of care.
• Assess and manage pain, symptoms and comfort needs.
• Provide skilled nursing care in the patient’s residence.
• Educate and support patients, families and caregivers.
