Job summary

Center4Hospice in Mishawaka, IN seeks an experienced Registered Nurse to join our hospice team as Case Manager. You will provide high-quality, in-home patient care and collaborate with an interdisciplinary team to honor the patient and family goals.

Responsibilities include comprehensive assessments, care planning, pain and symptom management, documentation, staff supervision, and education for patients and families. Monday–Friday day shift with on-call rotation.

Qualifications

• Must have Indiana RN license.
• Experience in hospice or home health preferred.
• Excellent communication and interdisciplinary collaboration skills.

Responsibilities

• Performs initial and ongoing comprehensive assessments within required timeframes.
• Develops individualized care plans with patient/family goals.
• Assesses pain and comfort, implementing management plans.
• Provides patient/family-centered care in the patient’s residence.
• Supervises LPNs and Home Health/Hospice Aides as needed.
• Performs admissions and maintains thorough documentation.
• Completes ~25 visits per week and participates in IDG meetings.
