Job summary

Puget Sound Home Health and Hospice in South King County, Washington is hiring a full-time Hospice Registered Nurse Case Manager (RN) to lead patient care for home hospice patients. You will perform assessments, update care plans, administer meds, and coordinate with physicians and other care team members to ensure high-quality end-of-life care.

Qualifications

• Ability to work independently and demonstrate critical thinking.
• Registered Nurse (RN) licensure.
• Current BLS certification from AHA.
• Be a licensed driver with an insured automobile in good working order.

Responsibilities

• Perform patient assessments (initial + ongoing).
• Develop and update care plans.
• Administer meds and manage symptoms.
• Educate/support patients and families.
• Attend/participate in IDT meetings.
• Make home visits as needed.
• Document in EMR accurately and timely.
• Communicate with physicians and team.
• Triage urgent calls/on-call rotation.
• Guide on hospice philosophy/DNR/advance directives.
• Provide end-of-life emotional support.
• Mentor new staff.
• Ensure compliance with regulations.
• Coordinate with outside services (DME, pharmacy, etc.).
