Job summary

Envision Hospice in Washington state is seeking a dedicated Hospice Registered Nurse to provide compassionate end‑of‑life care across Kitsap County. Care is delivered in homes, adult family homes, assisted living communities, and skilled nursing facilities, with a patient‑centered focus on dignity and comfort.

You will assess needs, coordinate with physicians and the interdisciplinary team, administer medications, educate families, maintain thorough documentation, and ensure Medicare and state

Qualifications

• Current, active Washington State RN license in good standing.
• Active BLS certification.
• Valid driver's license and reliable transportation.
• Minimum 1 year nursing experience in hospice or home health preferred.
• Minimum 2-3 years of healthcare related experience required.
• Proficient with EMR systems and healthcare technologies.
• Ability to exercise sound judgment and work independently within scope of practice.
• Adaptable to diverse social, cultural, and religious backgrounds.

Responsibilities

• Perform initial and ongoing assessments and develop care plans.
• Collaborate with physicians, social workers, volunteers, and team members to implement care plans.
• Administer medications and therapeutic interventions to manage pain and symptoms.
• Educate patients and families on hospice philosophy, disease progression, and end‑of‑life expectations.
• Provide emotional support to patients and families experiencing grief or anxiety.
• Maintain accurate, timely clinical documentation reflecting care provided and patient changes.
• Ensure care delivery complies with Medicare/state regulations and hospice policies.
• Advocate for patient dignity, preferences, autonomy, and rights.
• Respond promptly to urgent patient needs and collaborate with the on‑call team.
• Participate in quality assurance, performance improvement, and interdisciplinary team meetings.
• Participate in scheduled weekend rotations.
