Job summary

The Pennant Group, Inc. is hiring a full-time Hospice Registered Nurse Case Manager for the King County, WA area. You will conduct initial and ongoing patient assessments, develop care plans, administer medications, and support families in end-of-life care.

Strong hospice experience preferred; must hold RN licensure and a current BLS certificate. A valid driver's license with insured vehicle is required. The position offers comprehensive benefits and competitive compensation in the Puget Sound

Qualifications

• RN licensure in good standing.
• Hospice agency experience strongly preferred.
• Ability to work independently and demonstrate critical thinking.
• Current BLS certification from AHA.
• Licensed driver with insured automobile in good working order.

Responsibilities

• Perform patient assessments (initial + ongoing).
• Develop and update care plans.
• Administer meds and manage symptoms.
• Educate and support patients and families.
• Attend/participate in IDT meetings.
• Make home visits as needed.
