Job summary

Puget Sound Home Health and Hospice in Federal Way is seeking a compassionate Hospice Registered Nurse Case Manager (RN) for full-time duties in King County. Qualifications

• RN license required in good standing.
• Hospice experience preferred (case management ideal).
• Current BLS certification from an approved provider.

Responsibilities

• Perform patient assessments (initial and ongoing).
• Develop and update individualized care plans.
• Administer medications and manage symptoms.
• Educate and support patients and families.
• Attend and participate in IDT meetings.
• Make home visits as needed to monitor progress.
• Document in EMR accurately and timely.
• Communicate with physicians and team members.
• Triage urgent calls and participate in on-call rotation.
• Guide hospice philosophy, DNR, and advance directives.
• Provide end-of-life emotional support and mentorship to staff.
• Ensure regulatory compliance and coordinate with outside services.
