Job summary

Transitions Hospice is seeking an RN Case Manager to coordinate comprehensive hospice care for patients in Mattoon, Charleston, and surrounding areas. You'll lead care planning, collaborate with physicians and the care team, and serve as the main point of contact for families.

The role requires an active RN license and nursing degree, with hospice/home health experience preferred. Expect a rotating schedule with 4-day and 3-day weeks and weekends, plus excellent benefits and a supportive team.

Qualifications

• Active RN license in the state of employment is required.
• Graduate of an accredited school of Nursing.
• One year of hospice, home health, or related experience preferred.

Responsibilities

• Coordinate care as the clinical anchor for patients and families.
• Guide the interdisciplinary team and ensure timely, accurate documentation.
• Administer medications and treatments per physician orders.
• Educate patients and caregivers to support end-of-life care decisions.
• Serve as the communication bridge between families, physicians, and community partners.
• Participate in on-call rotation as required by team policy.
