Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our hospice-focused team delivering compassionate care across patient homes in Missouri. You will assess needs, monitor patient conditions, and educate patients, families, and staff about the plan of care and hospice philosophy.

You’ll work with physicians and care teams to respond to changes, support bereavement processes, and ensure safe, high-quality care while maintaining flexibility to travel between patient homes and assigned

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing school (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of nursing experience in a clinical setting.
• Ability to lift up to 50 pounds and assist with transfers.
• Driver's license or reliable transportation to travel to patient homes.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families for physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and report significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff on the Plan of Care.
• Provide education on hospice philosophy and symptom management to staff in various care settings.
• Provide compassionate support at end of life and complete bereavement assessments.
