Job summary

Elara Caring is seeking a Registered Nurse Case Manager to support hospice patients in home settings. You will assess needs, coordinate care with an interdisciplinary team, and guide families through care plans in their own homes. This salaried role offers the chance to impact patient outcomes across the community.

You will report to the Clinical Team Manager and travel up to 50% within the service area. A nursing license and clinical experience in hospice or home health are required or

Qualifications

• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• Current State License as a Registered Nurse (RN).
• Minimum 1 year of experience in a clinical care setting.
• Hospice or home health experience preferred.
• Travel required up to 50%.
• Ability to lift/push/pull up to 50 pounds and transport equipment.

Responsibilities

• Assess hospice patients and families to identify physical, psychosocial, and environmental needs.
• Communicate findings and changes in health condition to the Clinical Supervisor, physician, and care team.
• Counsel and involve patient and family in the Interdisciplinary Plan of Care.
• Educate staff about hospice philosophy, pain and symptom management.
• Provide support at time of death and perform bereavement assessment.
