Lighthouse Hospice NJ is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to deliver hands-on hospice nursing care across Mercer, Burlington, Camden, Gloucester, and Salem counties. The role focuses on coordination, education, and compassionate support for patients and families.

The ideal candidate holds an active RN license with at least 1 year of clinical experience, EMR proficiency, CPR certification if required, and reliable transportation.

• Hold current unencumbered license as a Registered Nurse.

• Respond to clinical referral information with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide hands‑on hospice nursing care and coordinate services.
• Deliver nursing care to ease pain and improve quality of life.
• Educate patients and families to empower active participation in care plans.
• Participate in on-call duties and respond to caregiver needs.
