Job summary

Lighthouse Hospice, Inc. in New Jersey is seeking a dedicated Hospice Registered Nurse Case Manager (RNCM) to join our compassionate team delivering end-of-life care across Mercer, Burlington, Camden, Gloucester, and Salem counties.

This full-time role offers a Monday–Friday schedule with day hours and a focus on delivering comfort and support to patients and families. You will coordinate services, educate families, and collaborate with the Clinical Director to ensure the right care at the right

Qualifications

• Must hold an unencumbered RN license.
• Minimum 1 year in medical-surgical/acute care; hospice experience a plus.
• Strong computer skills and EMR experience.
• CPR certification if state requires.
• Reliable transportation.

Responsibilities

• Deliver hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility for services.
• Support patients and families to ensure comfort and quality of life.
• Provide call coverage as part of the team.
