Job summary

Lighthouse Hospice, Inc. is seeking a full-time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team delivering expert end-of-life care across Mercer, Burlington, Camden, Gloucester and Salem counties.

Schedule is Monday–Friday, 8:00 am–4:30 pm, with a focus on comfort, dignity, and clinical excellence. As an RN Case Manager you will coordinate services, educate families, and support patients through the hospice journey.

Qualifications

• RN license active in good standing.
• 1 year or more in medical-surgical/acute care; hospice experience a plus.
• Computer proficiency including electronic medical records (EMR).
• CPR certification current if required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise to ensure exceptional service.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct nursing care and coordinate services to maintain patient comfort and quality of life.
• Deliver nursing with compassion and maintain patient-centered care.
• Educate patients and families to empower participation in personalized care plans.
• Participate in on-call duties as part of the team.
