Job summary

Vitality Hospice & Palliative Care is hiring a Registered Nurse Case Manager (RNCM) in New Jersey. This full-time position is Monday–Friday, 8:00 AM–5:00 PM, with occasional evenings and weekends. The role involves patient admissions, nursing assessments, and creating individualized care plans in collaboration with the IDT.

Travel to patient residences is required to deliver compassionate hospice care. The ideal candidate holds an NJ RN license, has hospice/home health experience, and

Qualifications

Current RN license in New Jersey required.
Hospice or home health experience preferred.
Excellent communication and organizational skills.
Valid driver’s license, reliable vehicle, and auto insurance.

Responsibilities

Conduct patient admissions and ongoing nursing assessments.
Develop and implement individualized plans of care based on patient needs and goals.
Collaborate with hospice physicians and the interdisciplinary team (IDT).
Supervise and coordinate care with home health aides, social workers, and chaplains.
Educate patients and caregivers about disease progression and symptom management.
