Qualifications

• Must have a current unrestricted RN licensure in Louisiana.
• At least 1 year of clinical experience as a registered nurse.
• CPR certification is required.
• Must possess a valid driver's license and vehicle insurance.

Responsibilities

• Conduct initial nursing evaluations within 48 hours of referral.
• Identify and reassess patient/family needs every 15 days.
• Coordinate and document the plan of care.
• Collaborate daily with the patient and care team.

Skills

Current unrestricted RN licensure in the state of practice

1+ years of clinical experience

Current CPR Certification

Dependable vehicle access or public transportation
