You will take full nursing responsibility for the delivery of care and collaborate closely with patients, families, and healthcare professionals. The role requires at least 1 year of clinical experience, valid RN licensure, and strong communication skills. Qualifications

• Must be able to work independently.
• Ability to function in any home situation regardless of client demographics.
• 2+ years of full-time RN experience or equivalent hospice experience.

Responsibilities

• Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
• Coordinate and document the plan of care.
• Collaborate daily with patient/family and medical team.

Current and unrestricted RN licensure in the state of practice

1+ years of clinical experience

Current CPR Certification
