Responsibilities
Conduct initial nursing evaluation within 48 hours of referral.
Identify and reassess patient/family needs at least every 15 days.
Initiate and coordinate the plan of care.
Document goals, interventions, and patient/family responses.
Collaborate daily with the care team.
