Qualifications

• 1+ years of clinical experience required.
• Ability to function in any home situation regardless of client characteristics.
• Current driver's license and vehicle insurance needed.

Responsibilities

• Conduct initial nursing evaluation within 48 hours of referral.
• Identify and reassess patient/family needs at least every 15 days.
• Initiate and coordinate the plan of care.
• Document goals, interventions, and patient/family responses.
• Collaborate daily with the care team.
• Instruct patients and families in self-care techniques.
• Maintain accurate clinical notes.
