Job summary

Corewell Health Hospice is seeking a Hospice Nurse Case Manager in Grand Rapids. Under the Nursing Manager's supervision, you will coordinate end-of-life care, assess patient needs, develop individualized care plans, and collaborate with an interdisciplinary team to ensure comfort and symptom management.

You will travel to patients' homes and facilities, document care, educate families, and participate in safety and compliance initiatives.

Qualifications

• Associate's degree in nursing is required.
• 2 years of relevant experience.
• Michigan RN license upon hire.
• State of Michigan driver's license upon hire.

Responsibilities

• Coordinate and deliver end-of-life care and symptom management.
• Assess patient needs and develop individualized care plans.
• Collaborate with an interdisciplinary team to ensure quality hospice care.
• Provide patient and family education on disease progression and care options.
• Document care accurately and maintain compliant records.
• Travel to patients' homes, nursing facilities, and hospitals for direct care.
• Participate in safety initiatives and continuous professional development.
