Corewell Health is seeking a Hospice RN for the Grand Rapids area to provide end-of-life care and symptom management. The role supports patients at home, in nursing facilities, and in the hospital, under Nursing - Hospice West.

Typical schedule is Day shift, 8 a.m. to 4:30 p.m., Monday through Friday with on-call weekends, and 40 hours per week in a full-time capacity. Strong teamwork and compassionate care are essential.

Qualifications

• Associates degree in Nursing and MI RN license.
• Driver's License valid in Michigan.
• Bachelor's in Nursing preferred.

• Assess and implement the patient\'s treatment plan.
• Monitor and prevent physiological complications.
• Collaborate with interdisciplinary team to coordinate care.
• Provide end-of-life care and symptom management in home, facility, or hospital.

Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. About Hospice

In collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.

Scope of work

Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
