Job summary

Corewell Health Hospice in Grand Rapids is seeking a Registered Nurse to assess and coordinate end-of-life care for patients and families. You will work within an interdisciplinary team, develop and adjust care plans, and advocate for pain and symptom management in home, facility, and hospital settings.

This full-time role includes day shift and a 36-hour workweek. Responsibilities include supervising LPNs and CNAs, administering medications as ordered, and participating in safety initiatives

Qualifications

• Associate’s degree in nursing is required.
• Minimum 1 year of hospice experience preferred.
• Michigan RN license upon hire is required.
• CPR certification within 90 days of hire (AHA).

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate end-of-life care strategies for patients and families.
• Develop and adjust an interdisciplinary plan of care with input from the patient/family.
• Advocate for pain and symptom management.
• Educate patients and families on care issues and resources.
• Direct and supervise LPN and CNA staff.
• Administer medications and treatments as ordered.
• Identify and respond to emergencies and participate in safety initiatives.
• Attend shift reports and IDT meetings.
