Job summary

Corewell Health Hospice seeks a Nurse Case Manager to coordinate end-of-life care and symptom management for patients and families. Under Nursing Manager supervision, you will assess needs, create care plans, and work with an interdisciplinary team to deliver high-quality hospice care.

Responsibilities include home visits to patients and facilities, education for patients and families, and thorough documentation to ensure compliance.

Qualifications

• Associate’s degree in nursing; Michigan RN license on hire.
• 2 years of relevant nursing experience.
• Michigan Driver’s license on hire.
• Bachelor’s degree preferred.
• 1 year of relevant experience preferred.

Responsibilities

• Assess patients and develop individualized care plans.
• Administer medications and monitor symptoms.
• Collaborate with physicians, social workers, chaplains, and the care team.
• Educate patients and families on disease progression and care options.
• Provide emotional and psychosocial support to patients and families.
• Maintain accurate documentation and comply with regulations.
• Travel to patients' homes, nursing facilities, and hospitals for direct care.
• Seek opportunities for ongoing professional development in hospice care.
• Perform other duties as assigned.
