Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for Windsor, Vermont. This full-time Hospice RN role involves visiting clients at home across Windsor, Orange, and Caledonia counties with NO weekends and NO on-call requirements.

Responsibilities include medication administration, wound care, and documenting doctors' orders, with travel to cases as assigned. RN license in VT/NH and at least 1 year of clinical experience preferred.

Qualifications

• Current RN license in VT and NH.
• Minimum of one year of clinical (medical/surgical) experience.
• Prior home health or hospice experience preferred but not required.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and comfort care.
• Monitor clients' conditions and report changes to supervisor.
• Follow up on doctors' orders and document them properly.
• Perform client assessments as necessary.
• Occasional on-call after hours and/or weekends.
