Job summary

BAYADA Home Health Care in White River Junction, VT is seeking a Registered Nurse (RN) Case Manager for Hospice on a full-time basis. This role supports hospice clients in Windsor, Orange, and Caledonia counties, with no weekends or on-call requirements.

The RN will perform home visits, administer medications, wound care, and comfort measures while documenting orders and coordinating with the care team. Travel to assigned cases is expected, with competitive benefits and weekly pay.

Qualifications

A current license as a Registered Nurse (RN) in VT and NH.
A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
Prior home care and/or hospice experience strongly preferred, but not required.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.
Ability to travel to cases as assigned.

Responsibilities

Making home visits to clients in designated geographic territories.
Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
Following up with, executing, and properly documenting doctors' orders.
Performing client assessments as necessary.
Occasional on call after hours and/or weekends
