Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager for Hospice in Lebanon, New Hampshire. This full-time role serves hospice clients in VT/NH with no weekends and no on-call on a regular basis.

As part of a multidisciplinary team, you will provide palliative care at home, with benefits including medical, dental and vision coverage, mileage reimbursement, weekly pay, 401(k) with company match, and opportunities for career advancement.

Qualifications

A current RN license in VT and NH.
Minimum one year of recent clinical (medical/surgical) experience.
Home care and/or hospice experience preferred but not required.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.

Responsibilities

Make home visits to clients in designated geographic territories.
Administer medications, wound care, treatments, and comfort care as assigned.
Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
Follow up with and document doctors' orders accurately.
Perform client assessments as necessary.
Occasional on-call after hours and/or weekends.
