Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for a nursing job in Norwich, Vermont. The role involves home visits, medication administration, wound care, and coordinating with a multidisciplinary team to provide compassionate palliative care at clients’ homes.

This full-time Hospice RN position offers no weekends and no on-call requirements, with standard hours 8:30-5, Mon-Fri, and weekly pay. Prior hospice/home care experience is preferred but not mandatory.

Qualifications

• Current VT/NH RN license is required.
• Minimum of 1 year of clinical experience.
• Prior home care or hospice experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medication, provide wound care, treatments, and comfort care.
• Monitor clients' conditions and report changes to management.
• Follow up with and document doctors' orders accurately.
• Perform client assessments as necessary.
• Occasional on-call after hours and/or weekends.
