Job summary

Karen Ann Quinlan Hospice-PA is seeking a Registered Nurse Case Manager for visiting hospice services in Milford, PA. This full-time, dayshift position runs Monday through Friday from 8:30 a.m. to 4:30 p.m., with occasional on-call duties.

Compensation is $87,750 per year plus a $8,000 sign-on bonus paid over 1 year. You will perform ongoing nursing assessments, coordinate care with the interdisciplinary team, educate patients and families, manage medications, complete charting within 48 hours,

Qualifications

• RN with PA/NJ license and nursing degree.
• BSN preferred.
• CPR certification preferred.

Responsibilities

• Explain and coordinate patients' rights and hospice care plans.
• Perform ongoing nursing assessments of Hospice patients.
• Document nursing interventions and patient responses within 48 hours.
• Collaborate with the interdisciplinary team on care plans.
• Provide patient/family teaching and support.
