Job summary

Caring Hospice is seeking a dedicated Registered Nurse Case Manager to evaluate and update patient care plans, perform regular assessments, and maintain thorough documentation. You will coordinate with families and caregivers to ensure compassionate, compliant care.

Requirements include an active RN license in Pennsylvania, appropriate nursing degrees, and a minimum of two years in Med-Surg, Home Care, or Hospice. On-site role with benefits and a signing bonus.

Qualifications

• RN license in Pennsylvania is required.
• Bachelor’s, Associate, or Master’s degree in nursing.
• Minimum 2 years in Med-Surg, Home Care, and/or Hospice.
• Ability to lift up to 25 lbs and maintain patient confidentiality.

Responsibilities

• Provide nursing care with emphasis on pain and symptom management.
• Supervise Home Health Aides.
• Refer patients and families to community resources.
• Maintain thorough and confidential patient records.
• Coordinate care directives with families and caregivers.
