Job summary

UnitedHealth Group is seeking an RN Case Manager to coordinate compassionate hospice care across Bourne, Falmouth, and Hyannis orientations. You will assess patients, collaborate with a multidisciplinary team, and develop care plans that respect each patient’s needs.

The role requires an active RN license for the state(s) of assignment, at least 1 year of nursing experience, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• Current RN license valid in the state(s) you will work.
• 1+ years of nursing experience.
• Current CPR certification.
• Valid driver’s license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness per center policies.
• Records pain and symptom management changes/outcomes as appropriate.
• Educates patients, families, and caregivers on appropriate care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services as assigned.
• Other duties as assigned.
