Qualifications

• Minimum one year of nursing experience.
• Current RN license for designated state.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform patient assessments and develop plans of care.
• Refer to Hospice care disciplines as needed.
• Maintain individualized patient care progress records.
• Record pain and symptom management changes.
• Educate patients, families, and caregivers.
• Communicate with clinical manager and care team.
• On-call responsibilities as assigned.
• Other duties as assigned.
